Discharge word/treatment is not recorded as full cost reimbursement assistance is not provided. Also avail the benefits for the following schemes eligible in the applicant state.
If the patient is a beneficiary of Mahatma Jyotiba Phule Jan Arogya Yojana / Abhiyaman Bharat National Health Program / Children / Charity in any other party, the application will not be accepted.
It is mandatory to attach documents/certificates related to the information shown in the application by the applicant self-attested (self-attested).
Applications furnished with questionable or false/false information shall be liable to cancellation as well as consequences.
The application must be accompanied by the following documents attested
Medical budget / total copy of the truth and certificate of learning expenses must be checked by a CV surgeon if a doctor.
Tehsil Income Base (less than Rs. 1.60 lakh office arrangement required.)
Patient Aadhaar Card (Maharashtra)
Patient Card (Maharashtra State)
Related relationship reporting is required.
Ensure Board records authorization officers on computer system.
